These terms govern your access to and use of the Optimal Wellness Clinic website. By using the site, you agree to these terms.

Website use, a form submission, or an appointment request does not by itself create a provider-patient relationship.

Emergencies and urgent concerns

This website is not monitored for emergencies. If you believe you are experiencing a medical emergency, call 911 or go to the nearest emergency department. If you or someone you know is in emotional distress or at risk of self-harm, call or text 988 in the United States.

Eligibility and clinical services

The website and app are intended for adults age 18 and older. Clinical services are available only where a clinic professional is appropriately licensed and after any required evaluation. No medication, treatment, laboratory test, or result is guaranteed to be appropriate for you, and outcomes cannot be guaranteed.

Appointments, costs, and cancellations

An appointment request is not confirmed until the clinic confirms it. Service availability and pricing may change. Applicable charges, payment expectations, cancellation terms, and any available Good Faith Estimate will be communicated separately. Most clinic services are private pay.

Acceptable use

  • Do not misuse the site, attempt unauthorized access, interfere with its operation, introduce malicious code, or use automated tools to scrape or overload it.
  • Do not submit information that is false, unlawful, infringing, threatening, or that violates another person's privacy.
  • Do not impersonate another person or schedule services without appropriate authority.

Ownership

The site design, copy, graphics, logos, photographs, and other content are owned by or licensed to Optimal Wellness Clinic and are protected by applicable intellectual-property laws. You may view and print reasonable portions for personal, noncommercial use, but may not reproduce, modify, distribute, sell, or exploit site content without written permission.

Third-party services

The site may link to or embed services operated by third parties, including Google. We do not control those services and are not responsible for their content, availability, security, or privacy practices. Your use of them may be governed by separate terms.

Disclaimers and limitation of liability

We work to keep site information useful and current, but provide the site on an “as available” basis and do not promise that it will always be complete, accurate, secure, or uninterrupted. To the fullest extent permitted by law, Optimal Wellness Clinic is not liable for indirect, incidental, special, consequential, or punitive damages arising from website use. Nothing in these terms limits rights or liability that cannot lawfully be limited.

Governing law and changes

These terms are governed by the laws of the State of Ohio, without regard to conflict-of-law rules. We may update them from time to time; continued use after an update means you accept the revised terms.
