Muscle loss on GLP-1s, and the two things that prevent it.
GLP-1 weight loss can include a meaningful share of lean mass alongside fat, especially without enough protein or resistance training. Adequate protein and two to three strength sessions a week are the two things that reliably protect muscle during the process.
Why GLP-1 weight loss isn't all fat
Any significant weight loss, on any protocol, includes some lean mass alongside fat — the body does not lose exclusively from fat stores just because the deficit came from an appetite-suppressing medication. Published research on GLP-1 medications has found that lean mass can make up a meaningful share of total weight lost when nothing is done to protect it, which is exactly why protein and resistance training are treated as part of the medical plan here, not an optional extra.
The two things that prevent it
Enough protein, every day, prioritized first. Protein preserves the signal your body needs to hold onto muscle even while running a calorie deficit. How much protein you actually need on a GLP-1 covers the specifics.
Resistance training, at least twice a week. Muscle that is being actively used is muscle the body is far less willing to break down for energy. Cardio alone does not send that signal — lifting something heavier than your daily routine does.
What a real week looks like
Two to three strength sessions a week, focused on the major muscle groups rather than isolated exercises, is enough to make a real difference for most people starting out. It does not need to be complicated or long — consistency across weeks matters far more than intensity in any single session. Signs you might be losing more muscle than you'd like
Feeling noticeably weaker in everyday tasks, clothes fitting looser everywhere including places that were already lean, or a scale number dropping fast with nothing to show for it in how you feel — these are worth a conversation with your coach, not something to wait out.
